                            Pearls & Oy-sters: Trigeminal Cystic Schwannoma Presenting With Foster Kennedy Syndrome, Sixth Nerve Palsy, and Focal Seizures. Foster Kennedy syndrome refers to a finding of optic atrophy in one eye from direct compression of the optic nerve by a mass lesion and contralateral papilledema in the non-atrophic optic nerve caused by increased intracranial pressure. When the fundoscopy finding is not due to a direct compressive mass, the term pseudo-Foster Kennedy syndrome is used; this can be caused by any process or processes that result in optic atrophy in one eye and optic disc edema in the other.                             retroperitoneal paragangliomas. A pituitary incidentaloma was found during investigations on a fluorodeoxyglucose (FDG)-positron emission tomography (PET) (FDG-PET). A pituitary magnetic resonance imaging (MRI) confirmed the presence of a 14 mm pituitary macroadenoma. The pituitary function was normal except for hypogonadotropic hypogonadism. On examination of the fundus, a diagnosis of Pseudo Foster-Kennedy syndrome was made due to a venous compression of the right jugular vein caused by the paraganglioma (PGL). The pituitary adenoma was not compressive to the optic chiasm. A treatment with acetazolamide was started in order to improve intracranial hypertension. Vision is rarely affected in acute papilloedema but peripheral vision may be lost in chronic cases where it is frequently accompanied by transient visual obscurations. Although Foster Kennedy is the name given to the syndrome localizing an intracranial tumour with respect to papilloedema, it is important to recognize how Victor Horsley was one of the strongest proponents in the usefulness of that sign. Not only did he describe the constellation of findings before Kennedy did, he was also probably one of the people who had influenced Kennedy the most during the latter's time at the National Hospital for Nervous Diseases at Queen Square.
